 Professional Geriatric Care Management Services

Professional Geriatric Care Managers provide a variety of services for seniors and their families.  A GCM first performs an assessment and then creates an “individual” care plan, which is a list of  recommendations of the best services and solutions for the senior. 

How GCMs Work: 

Knowing the details of how GCMs work will help you to know what to expect from an assessment as well as services.

An Initial Assessment:

Is an interview conducted with the senior in their residence.  If possible, it is helpful to have family members also present.  Telephone interviews or discussions are also possible.  The questions may range through issues of health, medications, daily activities, hygiene, cognitive thinking (memory and thinking skills), finances and more.

The Care Plan:  

Includes results of the assessment as well as short-term and long-term recommendations by the GCM.  Recommendations are provided in great detail and include names, numbers, and addresses of other professionals and services in the community that may fill the needs of the elder.  These may include homecare services, assisted living or long-term care recommendations, medical services, legal services, therapies, environmental and hygiene recommendations.

Continued Monitoring and Placement:

Assistance is provided should the senior or family require continued assistance or placement in an Assisted Living facility or a Nursing Home.   Recommendations are made and ongoing assessment by the GCM can be provided to monitor the senior’s status.  Regular visits are often scheduled to monitor a home or institution situation and offer a liaison to the family as a professional support provider of information.

Crisis Management Services:

We are able to quickly assess difficult and stressful situations and offer solutions and peace of mind. 

Transportation:

We provide transportation and supervision to medical appointments and other events.  We are able to provide valuable information to the senior’s physicians and also keep the senior’s family informed about ongoing medical issues.  We also help to coordinate care and information especially when there is more than one physician involved.

Home Health Care/Companion Services:

Can be arranged if needed and monitored to insure quality service.  We can find, hire and manage in home personal caregivers, and  keep family informed of their loved ones daily health and living conditions.

Medicaid Applications, Benefits and Entitlements Counseling:

Understanding what entitlements or benefits are available to you can be difficult.  We make this easier and can guide a senior or their family through this process.  When the time comes, we can provide some simple guidance and planning to maximize the allowable resources and complete the Medicaid application.  For more complex Medicaid planning, we are happy to refer you to a local Elder Law Attorney.

Money Management:

We assist with both on-site and in-office bill paying.  This often removes the burden of writing checks and balancing accounts.  We will act as Power of Attorney and or Health Care Proxy for a senior in special circumstances.
